2023 Molten Champion Basketball School of Queensland

Excursion Description:

As part of our Basketball program, your child has been selected to represent Capalaba State College in the Molten Champion Basketball School of Queensland held at the Gold Coast. The tournament will be from the 14th - 17th September 2023. The cost of the tournament will be $80 (non-refundable) which will cover team registration, individual registration, court fees, referees and incidentals. Students will be expected to comply with the terms of the College’s Responsible Behaviour Plan for students for the duration of this activity.

Gold Coast (Coomera Indoor Sports Centre & Runaway Bay Indoor Sports Centre)


Transport:

Students will need to be transported to and from the venue each day by a carer. 


Arrival/Return Times:

Departure Time: Time depending on the team’s first game in draw

Return Time: Time depending on completion of the team’s last game in draw


Activity Risk Level:

Medium


Activity Risk Management Strategies:

Students will be supervised by teachers for the duration of each of the games.

First Aid will be available at the venue


Dress code:

Students are to wear their full College sports uniform and will be required to wear their basketball jersey and matching shorts for matches. 


Other:

Students have trialled for this competition, so once payment is made, they are expected to be available for the entire weekend. 


Food – your child may need extra money for food which can be purchased at the venues during the day. Alternatively, food can be packed and taken to the competition in their bag.

  • The Cost of this excursion/ activity will be $80. 
  • This amount is non-refundable. 

Optional:

  • Long sleeve warm up jersey - $35
  • Personalised playing jersey and shorts - $80
  • Note – These will have to be arranged early to ensure they are ready in time.


Privacy Notice:

The Department of Education and Training (DET) is collecting the personal information requested as part of this excursion process in order to:

  1. obtain lawful consent for your child to participate in the activity;

  2. help coordinate the activity;

  3. respond to any injury or medical condition that may arise during, or as a result of the activity; and

  4. update school records where necessary.

 

The information will only be accessed by authorised school staff and will be dealt with in accordance with the confidentiality requirements of as applicable, s.426 of the Education (General Provisions) Act 2006 (Qld) and the Information Privacy Act 2009 (Qld) and/or the Privacy Act 1988 (Commonwealth).

 

The information will not be disclosed to any other person or agency unless the disclosure is authorised or required by law, or you have given the Department permission for the information to be disclosed.

 

As excursions and camps are conducted away from school grounds, there is an increased duty of care, responsibility, and accountability placed on staff as there are fewer support resources available. Students who are deemed to have demonstrated regular, inappropriate, unacceptable and/or unsafe behaviours while in the capacity of a Capalaba State College student, may pose an unacceptable risk to the school’s obligations under duty of care. Decisions about whether a student will attend an excursion or camp will be made by the Principal in consultation with the Deputy Principal and Year Level Coordinator after conducting a risk assessment in consultation with the parent of the student. The student and their family will be advised accordingly after a decision has been made. Students who are unable to attend an excursion or camp will have alternative arrangements made for a program of work to be undertaken at school for the duration of the activity.

 

Students will be expected to comply with the terms of the Student Code of Conduct (2020 - 2023) for the duration of this activity.


Activity Risks & Insurance:

Please note that the Department of Education and Training (DET) does not have personal accident insurance cover for students. If your child is injured as a result of an accident or incident while participating in the activity, all costs associated with the injury, including medical costs are the responsibility of the parents/carers. Some incidental medical costs may be covered by Medicare. If you have private health insurance, some costs may also be covered by your provider. Any other costs must be covered by parents/carers. It is up to all parents/carers to decide what types and what level of private insurance they wish to arrange to cover their child. Please take this into consideration in deciding whether or not to allow your child to participate in this activity.

 

The school collected medical information about your child at enrolment. This information is stored electronically in OneSchool. Please indicate by selecting the most appropriate answer below as to whether you need to provide us updated medical information relevant to the student attending the excursion as this may effect your child's full participation in the activity described in the form.

 

Acknowledging

  1. I have read all of the information contained in this form in relation to the activity (including any attached material);

  2. I am aware that the department does not have personal accident insurance cover for students;

  3. I give consent for my child/student to participate in the identified activity;

  4. I will pay to the school the costs detailed in this consent form for your student's participation in the activity;

  5. I agree to and understand the refund policy as it applies to this excursion (see Activity costs);

  6. In the event of an accident or illness, school staff may obtain or administer any medical assistance or treatment the child/student may reasonably require, including contacting their doctor;

  7. I accept liability for all reasonable costs incurred by the department in obtaining such medical assistance or treatment (including any transportation costs) and undertake to reimburse the department the full amount of those costs;

  8. I have provided the school with all relevant details of the child/student's medical or physical needs on registration/enrolment and where relevant have updated this information;

  9. I give consent for student contact information to be shared in relation to this activity in compliance with relevant Queensland Chief Health Officer's Directions.
